NOTE: Birth Certificate can be issued if birth occurred anywhere in the state of Texas and a Death Certificate can only be issued if death occurred within Waller County.

 

** If applying for a Passport, you will need the long form

Birth Certificate from the county you were born in. **

The Waller County Clerk's Office can print remote birth certificates for the State of Texas, however, the remote birth certificates may not be accepted by the passport office.  If you are needing a birth certificate for a passport, you need to request the long form from the county where you were born.

 

 

Only a Qualified Applicant May Obtain a Copy of a Birth/Death Certificate

Qualified Applicants Include:

  • Yourself, the parent, child, brother, sister, spouse, grandparent, step-parent, step-sibling, legal guardian, or managing conservator.


 


The following information must be provided as well as Proper Form of Identification.

Birth Certificate:

  • Full Name on Birth Certificate
  • Date and Place of Birth
  • Father's Full Name
  • Mother's Full Name (Including Maiden Name)
  • Name, Address and Phone Number of Person Requesting the Copy
  • Relationship to Person on Birth Certificate
  • Purpose for Obtaining this Record

Funeral Homes: If you are in need of a burial transit permit at a time not within our normal office hours, please e-mail you request to d.hollan@wallercounty.us.

Please mail in:

  • A copy of your ID
  • The Notarized Proof of Identification
  • The Application completely filled out
    • Full Name of Person on Record
    • Date of Birth or Death (Month, Day, Year)
    • Place of Birth or Death (City, County, State)
    • Full Name of Father
    • Full Name of Mother (Including Maiden Name)

Person Requesting Birth/Death Certificate:

  • Name of Person Requesting Birth/Death Certificate
  • Address
  • Telephone Number
  • Relationship to Person (Must be Individual or Immediate Family Member)
  • Purpose for Obtaining this Record
  • A Copy of Your Valid Driver's License

Burial Transit Permit

Burial Transit Permits may be requested and printed through TxEver.  If you are unable to print your request through TxEver and need to obtain a burial transit permit during or after our normal business hours or on holidays please email the County Clerk’s office at s.tompkins@wallercounty.us AND j.deutrich@wallercounty.us.

After the request is sent through TxEver, please include the following in your email request:

  • Deceased person’s name 
  • Authorization of Next of Kin to Release Decedent/Personal Effects to Funeral Director – If the death is not natural and an autopsy was performed by the Fort Bend County M.E.’s office, we will need the Authorization of Next of Kin to Release Decedent/Personal Effects to Funeral Director from the M.E.’s Office that is signed.
  • Cremation Authorization – If no Autopsy was needed, we will need the Cremation Authorization that is signed by the JP.

 

NOTE: A BurialTransit Permit must be obtained for cremation. [CCP Art. 49.09, CCP Art. 49.25(10), 25 TAC 181.2] A body may not be cremated until 48 hours after death as indicated on the Certificate of Death unless the death certificate indicates the death was due to Asiatic cholera, bubonic plague, typhus fever, or small pox.
